Dr. Horn was a medical doctor from a line of presitgious doctors, among them his late father, and his identical twin.

His brother was disowned by their father for wanting to help the poor in the East End, however the true reason was that their father was a high-ranking member of the Royal Medical Society, and felt that his son would never have been able to follow the society's morally questionable creed.

Unlike his brother, however, Dr Horn was taken into the fold, and eventually assumed his father's position when he died.

He used his expertise in infectious diseases to cultivate mosquitos carrying the deadly Black Fever, and also manufactured a cure. As part of the Royal Medical Society's goal to both resurrect Lord Leslie's daughter, and access the secrets of the Émigré Manuscript, he supported his brother's endevours in the East End, planning to use his brother's medical reports to find suitable sacrifices for the ritual.

He employed the services of The Asian Man to abduct pregnant women, remove their foetuses, and preserve them alive in a special solution of his own devising.

However, while the Asian Man had successfully kindnapped Pacey Jones, his face had been seen by Koudelka Iasant. To force him into killing Koudelka, and to tie up any loose ends should he fail, Dr Horn deliberately infected him with Black Fever, and dangled the promise of a cure in front of him to keep him in line.

As the Asian Man and Koudelka battled in Covant Garden Cemetary, he watched from a distance through a telescope, alongside De Gaulle, who told him that Koudelka was the one who was responsible for the events at Nemeton Monastery.

De Gaulle explained the powers that she had, and that she was currently possessed by Snake Demons, and that what he was seeing was an illusion, and that being a doctor only interested in the physical realm, he wouldn't understand.

Koudelka overcame the possession, and Dr Horn question why they shouldn't kill her; Lord Leslie had given orders after all - only to be told that she may be useful in the long term. However, he was told he may do what his wished with the Asian Man.

He managed to scare off Koudelka and Joshua by pretending to be a cemetary watchman, so he could get close to the Asian Man. He poured spirits over the man to set him alight, but the Asian Man pulled the cords from the gas tank he was wearing, making an improvised explosive, intending to take the doctor out with him.

Unfortunately, Dr Horn anticipated this, and was wearing a protective suit of his own design, which shielded him from the blast.

Later, he performed a forced cesarean section on Pacey, and left her outside the Club Mary. But not before sending his brother a telegram telling him to head there. It was all part of his plan to get close to abduct Koudelka.

After his brother left the Club, he knocked out one of Inspector Dickens's policemen and pretended to be his brother returning, having forgotten some medical equipment.

While her back was turned he tried to inject her, but was forcefully restrained by the ghost of the Asian Man. Confused, he fled to a secret cellar where he conducted his grisley surgeries, only to be pursued by Koudelka, his brother, Joshua, and the others.

He was attempting to take as many of his books and notes as possible, and flee to the protection of the Royal Medical Society, where he could lie low for a while, but was interupted by the group.

He casually revealed what he had done to them, how he had used his brother, and how much he despised prostitutes, whom he saw as lowly, and boasted about how those foetuses would be used in sacrifice.

He threw Pacey's baby at them as a distraction while he suited up in his protective suit. Before activating a switch that would release modified Black Fever carrying mosquitos that would instantly infect them.

He was foiled by the sudden appearance of Dr Hans, who tore an opening in his suit, and gave everyone enough of a distraction to flee. Dr Horn was infected by the Black Fever and rushed towards a cabinet desperately seeking his antidote.

But what awaited him was one of De Gaulle's imp familiars sitting on an explosive, just before it exploded in his face, destroying the lab and killing Dr Horn.
